Interval Health History Form

Prior to the start of tryout sessions or practice at the beginning of each season, a health history review must be conducted for each athlete, unless he/she received a full medical exam within 30 days of the season’s start.

Health Exam Form

Prior to the start of tryout sessions or practice at the beginning of each season, every student must have a physical exam within the last 12 months. All athletes must schedule a physical with Bassett Healthcare, or they can choose to have their sports physical with their own physicians.

Late Bus Request

Students needing to use the late bus must have a parental Request for Late Bus Transportation on file at the Transportation Department.

Sports Alternative Pickup Note

If a parent prefers their student not ride the school bus for away games, they must complete a Sports Alternative Pickup Note.
